Patient is a 25-year-old male whose chief complaint is traumatic above-the-knee amputation from last MCI. ALS crew applies tourniquet and begins fluid resuscitation en route to the hospital. Crew member questions the patient quickly and appropriately, knowing the patient may soon lose consciousness due to hemodynamics instability. They obtain patient’s Past Medical History, and find him significant for malignant hyperthermia. During transfer of care to Ed/OR staff, crew member physically taped to the patient’s chest, a paper that reads “I have malignant hyperthermia.” Strong, effective communication results in anesthesia treating patient correctly and surgery yield successful results with full recovery.
Would you have asked the patient that specific question knowing they would go directly into surgery?
This patient would have died if regular anesthesia protocol had been followed.
